Buidling Usage Request Form

Ponca City Assembly reserves the right to deny the opportunity for building usage to any organization or person that adheres to practicies and beliefs that go against our constitution and bylaws.

All personal items will need to be removed directly following your event.

Janitorial services cost 100.00 and the staff of the Church will determine if it is required to restore the building depending on the date and time of your event

250.00 refundable security deposit is required to schedule your event. It will be refunded at the event has concluded.

For all non church ministry functions or events, unless otherwise stated a host/hostess is required to be on the property othe cost is 50.00 for 2 hours and 25.00 for additional hours required.

(Sound for the Sanctuary & Fellowship Hall requires a trained staff person)

(Requires a trained PCA Tech if showing a video or slideshow in the Sanctuary or Fellowship Hall)

Notes: For a wedding with reception (using fellowship hall and kitchen), you are renting the space ONLY. You will be responsible to bring everything you need for your event, including but not limited to your own cooking appliances (crock pots, ect.) utensils, dishes, ect.

Please Remember you are responsible for

1. Returning room(s) to condition it was found in

2. Placing all trash in the appropriate receptacles

3. Any damages to the property or theft of items

4. Communicating any changes to your itinerary with the staff of PCA Church.
